Any employee/spouse currently on our medical plan or who plans to enroll during open enrollment needs to complete a wellness screening, health risk assessment, and nicotine affidavit to receive the wellness incentive credits for the 2022 health plan year. All physician forms or LabCorp vouchers are due by December 17.
