Remember to keep your wellness screening appointment for the 2019 insurance plan year. Any employee and spouse on the medical plan (or who plans to enroll during open enrollment) needs to complete the biometric screenings and HRA (health risk assessment) to receive the wellness incentive credits for the 2019 plan year.  For those outside of the Indianapolis area, please contact Liza Trant at [email protected] for a physician’s packet or EScreen passport.

• Indianapolis – October 15, 16, 17, 18, 19 (7 a.m. to 11 a.m. all days)
• Highland – October 16 (8 a.m. to 11 a.m.)
• Columbus – October 17, 18 (8 a.m. to 11 a.m.)
• Evansville – October 18 (8 a.m. to 11 a.m.)
• Austin – October 18 (9 a.m. to 10:30 a.m.)
